The Manchester United left-back sickeningly hit the deck after colliding with Dani Carvajal’s arm during a mid-air challenge.
He remained motionless as six medics ran onto the Wembley turf.
A brace was placed around Shaw’s neck and he received oxygen before being taken off the pitch on the stretcher.
Jose Mourinho was watching on from the stands and will have been concerned by the distressing scenes.
Sky Sports pundit Gary Neville described Carvajal’s challenge as “reckless”.
Shaw was replaced by Tottenham left-back Danny Rose in the 53rd minute of the Nations League opener.
The 23-year-old is reportedly awake in the England dressing room and using his mobile phone.
Shaw was enjoying a sterling return to international football, having created Marcus Rashford’s opening goal in the first half.
England had the lead for less than 120 seconds, however, as Saul Niguez rounded off a fine move to level for Spain.

Shaw has been back to his best in recent weeks, nearly three years after breaking his leg in a Champions League clash against PSV Eindhoven.
Ahead of England vs Spain, Shaw admitted he nearly lost his leg as a result of his double break.
"I’d be lying if I said I didn’t sometimes think about stopping playing football," he said.
"But I had a lot of good people around me - friends and family who helped me through it.
"No one knows. I was really close to losing my leg.”
